To prepare injection from ampoules and vials
An ampoule contains an injectable medication in liquid form and is available in sizes from 1-10ml or more.
A vial is a single dose or multi dose container with a rubber seal at the top , protected by a metal or plastic cap . Vial contain liquid or dry forms of medication.

Procedure in a step wise manner
Check completeness of the MAR or computer prints out with prescriber written medication order
Check the patients records and medicational history
Review medication reference information related to medication , including action purpose, side effects, normal dose, rate of administration, time of pick onset and nursing implications.
Perform hand hygiene and preapares supplies
Check date of expiration
Select ampoule or vial from unit- dose drawer or automated dispensing system and compare lebel of medication with the medication record for the accuracy
Preparing an ampoule
Tap the top of the ampoule tightly and quickly until fluid from the neck of the ampoule
Place small gauze or unopened alcohol swab around the neck of the ampoule
Snap neck of the ampoule quickly and firmly
Draw a medication quickly using a filter niddle long enough of ampoule
Hold ampoule upside down or set in on a flat surface insert filter niddle into center of the ampoule opening . Don’t allow niddle tip or shaft to touch the rim of the ampoule.
Aspirate medication into syringe by gently pulling back on plunger
Keep niddle tip under surface of liquid . Tip ampoule to bring all fluids within reach of the niddle tip
If air bubbles are aspirated then don’t expell air into ampoule
To expell excess air bubble remove needle from ampoule . Hold syringe with needle pointing up. Tap side up the syringe to cause the bubbles to ride towards needle. Draw back slightly on plunger and then push plunger upward to eject air . Don’t eject fluid .
If syringe contains excess fluid use sink for disposal. Hold syringe needle with vertically with needle tip up and slanted slightly towards sink. Slowly eject excess fluid into sink. Recheck fluid level in syringe by holding in vertically.
Cover needle with it’s safety seath or cap. Don’t engaged the seath or it will lock.
Preparing a vial containing a solution
Remove cap covering top of unused vial to expose sterile rubber seal
With the vial on the flat surface , firmly insert the tip of the needle
Inject air into the vial air space , holding on to plunger hold plunger firm pressure
Invert vial keeping firm hold on syringe and plunger.hold vial between thumb and middle finger Of nondominat hand , grasp end of the syringe barrel and plunger with thumb and fourth finger of dominant hand to counteract the pressure in vial.
Keep tip of the needle in the fluid while withdrawing the medication
Allow in pressure from the vial to fill the syringe gradually with medication if necessary pull back slightly on plunger to obtain correct ammount of medication.
When desired volume has been obtained , positioned needle into vial air space tap side of the syringe barrel gently to dislodge any air bubble. Eject any air remaining at the top of the syringe into the vial.
Hold the syringe at the eye level at 90 degree angle to ensure correct volume and absence of air bubble.
Preparing a vial containing a powder
Remove cap covering the vial of powdered medication and cap covering vial of proper diluents. Firmly swab rubber seals with alcohol swab and allow alcohol to dry.
Draw of manufacturer’s suggestion for volume of diluents into syringe.
Insert tip of needle through to center of rubber seal of vial of powdered medication. Inject diluents into the vial. Remove needle.
Mix medications thoroughly roll in plans. Don’t shake
Reconstituted medication in vial is ready to be drawn into new syringe.read the lebel carefully to determine dose after reconstitution.
